- Disability/FMLA forms will be filled out for surgical patients only.
- Forms need to be faxed/emailed or dropped off to the office with the patient portion completed.
- There is a $25 fee each time forms are filled out or updated.
- We have a 3-5 day turnaround.
- When the forms are received, we will call/email you to go over any questions we have.
